ABSTRACT

Chronic hepatitis C (CHC) infection is a significant health problem that affects 185 million people worldwide. Complications such as fibrosis, cirrhosis, liver failure and liver cancer may also develop. The hepatitis C virus (HCV) may also progress with non-hepatic involvement. At least one extrahepatic manifestation (EHM) is observed in two out of three of these patients. EHMs may not always be seen together with liver disease findings, and may even appear before clinical chronic viral hepatitis findings. HCV can be diagnosed earlier by means of careful systematic examination. EHMs such as kidney diseases, autoimmune diseases, hematological diseases, diabetes and cardiomyopathy may be encountered in chronic HCV. EHMs can cause morbidity and mortality. Contemporary effective, oral, direct-acting antiviral (DAA) drugs are the first-choice medications. Sustained virological response (SVR) and a high success rate can be achieved in HCV patients with access to these drugs. This report discusses the current status of HCV infection, EHMs, and the available therapeutic options in the treatment of renal failure.

ÖZ

Kronik Hepatit C (KHC) infeksiyonu önemli bir sağlık problemidir. Dünyada 185 milyon insanı etkilemiştir. Hastalarda fibroz, siroz, karaciğer yetmezliği, karaciğer kanseri gibi komplikasyonlar gelişebilir. Özellikle hepatit C virüsü (HCV) karaciğer dışı tutulumlarla da seyredebilir. Bu hastaların üçte ikisinde en az bir extrahepatik bulgu (EHB) izlenmektedir. EHB her zaman karaciğer hastalığı bulguları ile birlikte görülmez. Hatta kronik viral hepatit klinik bulgularından önce EHB çıkabilmektedir. Dikkatli yapılan sistematik muayene ile HCV tanısı daha erken konulabilir. Kronik HCV’de böbrek hastalıkları, otoimmun hastalıklar, hematolojik hastalıklar, diabet, kardiomyopati gibi EHB ile karşılaşabiliriz. EHB morbidite ve mortalite nedeni olabilir. Güncel etkili, oral, güvenilir direkt etkili antiviral (DEA) ilaçlar birincil tercih edilen ilaçlardır. Bu ilaçlara ulaşabilen HCV hastalarında sürekli virolojik yanıta (SVR) ulaşılabilmektedir. Yüksek başarı oranı sağlanmaktadır. Bu raporda HCV infeksiyonun güncel durumu, ekstrahepatik bulgular, renal yetmezlikte güncel yeni tedavi tercihlerine değinilecektir.

The prevalence of HBsAg / HCV in Aksaray province, demographic data of patients with HCV and the rates of access to new treatments

ABSTRACT

Aim: Approximately 210 million peoples worldwide are estimated to be infected with chronic HCV. Nowadays, with the use of direct-acting antiviral agents, sustained viral response rates of more than 95% have been achieved in HCV. In our country, these drugs have been used for the last two years. The desired results could not be reached in the number of patients reaching treatment.
In this study, we investigated the prevalence of HBsAg and HCV in Aksaray. In addition, we aimed to reveal the demographic characteristics of patients infected with HCV, their awareness about the disease and the rates of access to treatment.
Material and methods: In this study, 11716 patients admitted to Aksaray Training and Research Hospital Gastroenterology, Infectious Diseases Outpatient Clinic and Endoscopy Unit between January 2016 and October 2018 were retrospectively screened. HCV positive patients were called from their contact numbers.
Results: HBsAg was detected in 244 (2.08%) and anti-HCV positivity in 71 (0.6%) of 11716 patients. Fifty-three patients with HCV were included in the study. 16 patients could not be reached. Nine of these patients were treated with new treatment regimens at the external center. Nine patients were reached during this study and were provided access to treatment. All of these patients were able to tolerate treatment and a permanent viral response was found.
Conclusion: Direct effective antiviral agents are effective and safe drugs in patients with HCV. Additional efforts and means of communication are needed to enable patients to access drugs.

ÖZ

Amaç: Tüm dünyada yaklaşık olarak 210 milyona yakın kişinin kronik HCV ile enfekte olduğu tahmin edilmektedir. Günümüzde doğrudan etkili antiviral ajanların kullanılmasıyla HCV’de tedavi başarısı %95’in üzerinde kalıcı viral yanıt oranlarına ulaşılmıştır. Ülkemizde de bu ilaçlar son iki yıldır kullanılmaktadır. Tedaviye ulaşan hasta sayılarında istenen sonuçlara ulaşılamamıştır.
Bu çalışmada Aksaray İlindeki HBsAg ve HCV prevalansını araştırdık. Ayrıca HCV ile enfekte olan hastaların demografik özellikleri, hastalıkla ilgili farkındalıklarını ve tedaviye ulaşım oranlarını ortaya çıkarmayı amaçladık.
Gereç ve Yöntem: Bu çalışmada Ocak 2016 ile Ekim 2018 tarihleri arasında Aksaray Eğitim ve Araştırma Hastanesi Gastroenteroloji, Enfeksiyon hastalıkları polikliniği ve endoskopi ünitesine başvuran 11716 hasta geriye doğru tarandı. HCV pozitif hastalar iletişim numaralarından arandı.
Bulgular: 11716 hastanın 244’ünde (%2,08) HBsAg ve 71’inde (%0,6) anti-HCV pozitifliği saptandı. Ulaşılabilen elli üç HCV’li hasta çalışmaya alındı. 16 hastaya ulaşılamadı. Bu hastalardan 9’unun dış merkezde yeni tedavi rejimleri ile tedavi aldıkları saptandı. Dokuz hastaya bu çalışma sırasında ulaşılarak tedaviye ulaşmaları sağlandı. Bu hastaların tamamının tedaviyi tolere edebildiği ve kalıcı viral yanıt sağlandığı tespit edildi.
Sonuç: HCV’li hastalarda doğrudan etkili antiviral ajanlar etkili ve güvenilir ilaçlardır. Hastaların ilaçlara ulaşımını sağlamak için ek çabalara ve bilgilendirmelere ihtiyaç duyulmaktadır.

Evaluation of hepatitis A, B and C serology in a second-stage state hospital employees

ABSTRACT

Aim: The most common occupational risks that health workers are exposed to infectious diseases. It is aimed to determine the seroprevalence of Hepatitis A, Hepatitis B and Hepatitis C from the healthcare personnel working at Yozgat City Hospital, which constitutes a high occupational risk for healthcare workers. Differences between age groups, gender, occupation, and working services were evaluated against these diseases.
Material and Method: The results of the health screening of the employees of our hospital were examined retrospectively. 628 health workers’ data have been reached through the hospital information system and the results were recorded for the age, gender, occupation, working unit, and anti HAV IgG, HBsAg, anti HBs, anti HCV.
Results: 628 health workers were included in the study, 378 (60.1%) participants were female and 250 of them (39.9%) were male. The average age of the participants is 35.8 ± 9.6. Anti HAV IgG positivity rates were determined to be 79.3% and 80.7% in females and 77.2% in males. Anti HAV IgG positivity in the group was evaluated as assistant health personnel and administrative staff was found statistically significantly higher than the doctors and nurses (p: 0.00). HBs Ag positivity was determined 1.3%, and anti HBs positivity was determined to 86%. Anti HBs positivity was found to increase with age (p: 0.00), in the physician and nurse group, immunizations were higher than assisted health personnel and administrative staff (p<0.05). Anti HCV positivity was detected in 0.5%.
Conclusion: The screening examinations of the staff at the hospital should be performed regularly and hepatitis A, which is usually neglected, should be included. It is required to the completion of hepatitis A and B vaccinations according to the screening results of all personnel, including healthcare personnel who do not contact with the patient, and for hepatitis C, universal precautions should be followed.

ÖZ

Amaç: Sağlık çalışanlarının en sık maruz kaldığı mesleki risklerin başında, enfeksiyon hastalıkları gelmektedir. Yozgat Şehir Hastanesi’nde çalışan sağlık personelinde, sağlık çalışanları için yüksek mesleki risk oluşturan hepatit A, hepatit B ve hepatit C seroprevalansının belirlenmesi amaçlanmıştır. Bu hastalıklara karşı, yaş grupları, cinsiyet, meslek ve çalışılan birimler arasındaki farklılıklar irdelenmiştir.
Gereç ve Yöntem: Hastanemiz personeline yapılan sağlık taraması sonuçları retrospektif olarak incelenmiştir. 628 sağlık çalışanının verilerine, hastane bilgi sistemi üzerinden ulaşılmış ve personellerin, yaşı, cinsiyeti, mesleği, çalıştığı birim ve anti HAV IgG, HBsAg, anti HBs, anti HCV sonuçları kaydedilmiştir.
Bulgular: Çalışmaya 378 (%60,1)’i kadın, 250 (%39,9)’u erkek olmak üzere 628 sağlık çalışanı dahil edilmiştir. Katılımcıların yaş ortalaması 35,8±9,6’dır. Anti HAV IgG pozitiflik oranları %79,3 saptanmış olup kadınlarda %80,7; erkeklerde %77,2 olduğu görülmüştür. Yardımcı sağlık personeli ve idari personel olarak değerlendirdiğimiz grupta anti HAV IgG pozitifliği, doktor ve hemşirelere göre istatistiksel olarak anlamlı derecede yüksek bulunmuştur (p:0,00). HBsAg pozitifliği %1,3; anti HBs pozitifliği %86 saptanmıştır. Anti HBs pozitifliğinin yaş ile birlikte arttığı (p:0,00), doktor ve hemşire grubunda bağışıklığın, yardımcı sağlık personeli ve idari personele göre daha yüksek olduğu görülmüştür (p<0,05). Anti HCV pozitifliği %0,5 saptanmıştır.
Sonuçlar: Hastanede çalışan personelin tarama muayenelerinin düzenli olarak yapılması ve genellikle ihmal edilen Hepatit A’ya yer verilmesi gerekmektedir. Hasta ile temas etmeyen sağlık çalışanları da dahil olmak üzere tüm personelin tarama sonuçlarına göre hepatit A ve B aşılarının tamamlanması, hepatit C için evrensel önlemlere uyulması gereklidir.

Frequency of occult hepatitis B and C infections in hemodialysis patients

ABSTRACT

Amaç: Hepatit B virüsü (HBV) ve Hepatit C virüsü (HCV) diyaliz ünitelerinde sıklıkla bulaşabilen virüslerdendir. Okült HBV veya HCV enfeksiyonu serumda HBsAg veya anti-HCV saptanamayan kişilerdeki HBV veya HCV enfeksiyonunun varlığının kan veya karaciğer dokusunda moleküler yöntemlerle (HBV-DNA, HCV-RNA ile) gösterilmesidir. Bu çalışmada hemodiyalize giren, HBV ve HCV seronegatif hemodiyaliz hastalarına ait plazma örneklerinde HBV-DNA ve HCV-RNA tayini ile okült HBV ya da HCV enfeksiyonu sıklığının araştırılması amaçlandı.
Gereç ve Yöntem: Çalışmaya hemodiyalize giren, HBV ve HCV göstergeleri (HBsAg, HBcIgG ve IgM , Anti-HCV) negatif olan 121 erişkin hasta dahil edildi. Hastalar hemodiyaliz süresi, kan transfüzyonu öyküsü, ailede hepatit B ve C enfeksiyonu varlığı açısından sorgulandı. Hastalardan alınan plazma örneklerinde HBV-DNA ve HCV-RNA varlığı COBAS® TaqMan® 48 Analyzer (for automated real-time PCR amplification and detection) kullanılarak COBAS Taqman kitleri ( Cobas Taqman HBV v.2 ve HCV v.2 Roche, US) kiti ile kantitatif olarak araştırıldı. .
Bulgular: Toplam 121 hemodiyaliz hastasının 1’inde (%0.82) HCV-RNA pozitifliği saptanırken (138.000 IU/ml ), 10(%8.2)’unda HBV-DNA saptanabilir düzeyde idi. HBV-DNA saptanan 10 plasma örneğinin 7 (%70)’sinde HBV-DNA düzeyi < 6IU/ml iken 3 (%30)’ünde HBV-DNA düzeyi > 6IU/ml olarak bulundu. HBV-DNA düzeyi > 6 IU/ml olan 3 hastaya ait HBV-DNA miktarları 108 IU/ml, 157.000.000 IU/ml ve 72.5 IU/ml olarak saptandı.
Sonuç: Çalışmamızda Hepatit B ve C yönünden serolojik göstergeleri negatif olan hemodiyaliz hastalarında okült hepatit B enfeksiyonu sıklığı okült hepatit C enfeksiyonundan yaklaşık 10 kat daha fazla oranda saptandı. Serumda, veya karaciğerde HBV-DNA veya HCV-RNA pozitifliği hemodiyaliz ünitelerinde potansiyel bulaş kaynağıdır. Hemodiyaliz hastalarında HBV veya HCV’ye bağlı okült hepatit varlığı diğer hastalara ve sağlık personeline bulaş olasılığı ve böbrek naklinden sonra hastalarda özellikle hepatit reaktivasyonu veya kronik karaciğer hastalığı gelişmesi açısından önemlidir. Bu nedenle özellikle transplantasyon planlanan hemodiyaliz hastalarında okült hepatit varlığı moleküler yöntemlerle araştırılmasının yararlı olacağı görüşündeyiz.

ÖZ

Aim: Hepatitis B (HBV) virus and Hepatitis C virus (HCV ) are commonly transmitted viruses in dialysis units. Occult HBV or HCV infections are HBV or HCV infections in patients when HBs Ag or anti-HCV is not detected in serum. In this study, it was aimed to detect HBV-DNA and HCV-RNA in plasma samples belonging to hemodialysis patients who are seronegative for HBV and HCV and to investigate frequency of occult HBV or HCV infections.
Material and Method: 121 adult patients who are receiving hemodialysis and whose HBV and HCV indicators (HBsAg, HBcIgG and IgM, Anti-HCV) are negative were included in the study. Presence of HBV-DNA and HCV-RNA in plasma samples of patients was investigated by using COBAS® TaqMan® 48 Analyzer (for automated real-time PCR amplification and detection) in a quantitative fashion with COBAS Taqman kits (Cobas Taqman HBV v.2 and HCV v.2 Roche, USA)
Results: In 1 out of 121 hemodialysis patients (0.82%), HCV-RNA level was detected as 138.000 IU/ml, in 10 out of 121 hemodialysis patients (8.2%) HBV-DNA was at detectable level. While in 7 of 10 plasma samples (70%) in which HBV-DNA was detected, HBV-DNA level was < 6IU/ml, in 3 of 10 plasma samples (30%) HBV-DNA level was found as > 6IU/ml. HBV-DNA amounts belonging to 3 patients whose HBV-DNA level was > 6 IU/ml were found as 108 IU/ml, 157.000.000 IU/ml and 72.5 IU/ml.
Conclusion: It was found out that in hemodialysis patients whose serologic indicators for Hepatitis B and C are negative, occult Hepatitis B infection is seen 10 times more frequently than Hepatitis C infection. Positive demonstration of HBV DNA or HCV RNA in serum or liver is potentially seen in hemodialysis units. Incidence of occult hepatitis due to HBV or HCV in hemodialysis patients is of significant importance in terms of the probability of its transmission to other patients and healthcare staff and development of especially hepatitis reactivation or chronic liver disease in patients after kidney transplant.
For this reason, we believe it would be particularly useful to investigate the presence of occult hepatitis by molecular methods in hemodialysis patients especially scheduled for transplantation.

Determination of hepatitis C virus genotype distribution in Adıyaman province

ABSTRACT

Aim: Hepatitis C Virus (HCV) infection is one of the most important liver diseases in Turkey and worldwide. HCV shows high degree of genetic heterogeneity; consequently six major genotypes and multiple subtypes of HCV have been identified so far in world. Distribution of HCV genotypes and subtypes in different regions of the world is variable. The objective of this study was to determine the distribution of genotypes of hepatitis C virus in patients with chronic hepatitis C infection in Adıyaman province.
Material and Method: Serum samples from 71 patients (26 female, 45 male) with chronic HCV infection were analyzed in this study. Viral genotypes were determined by using squences analysis (ABI Prism 310 Genetic Analyzer) and HCV-RNA Real Time PCR genotyping performance panel (PHW 202, BBI Diagnostics) working according to the manufacturer’s instructions.
Results : Genotype 1b was observed in 51 of the 71 patients (71.83%). The remainder of HCV-RNA positive specimens; In 6 patient was showed infection with subtype 1a (8.45%) and 8 with subtype 2b (11.27%). The rest of 6 specimen were determined genotype 1 and subtip 3a as 4.22%. There were no statistical differences between female and male patients groups for distrubition of genotype or subtype.
Conclusion: According to our results, we found genotype 1b was dominant in Adiyaman province of Turkey. Our results were similar with others performed in this area. In addition, dominant HCV genotype was 1b; however, the other genotypes have been increasingly determined.

Amaç: Hepatit C virus (HCV) enfeksiyonu Türkiye’de ve dünyada en önemli karaciğer hastalıklarından biridir. HCV yüksek düzeyde genetik heterojenite göstermektedir. Tüm dünyada şimdiye kadar altı majör tipi ve çok sayıda alt tipi tanımlanmıştır. Dünyanın farklı bölgelerinde HCV genotipleri ve alt tiplerinin dağılımı değişmektedir. Bu çalışmanın amacı Adıyaman ilinde kronik HCV enfeksiyonlu hastalarda HCV genotip dağılımını belirlemektir.
Materyal ve Metod: Bu çalışmada kronik HCV enfeksiyonlu 71 (26 kadın, 45 erkek) hasta değerlendirildi. Viral genotipler dizi analizi (ABI Prism 310 Genetic Analyzer) ve HCV-RNA Real Time PCR genotipleme performans paneli (PHW 202, BBI Diagnostics) ile üretici firmanın önerilerine göre çalışılarak belirlendi.
Bulgular: 71 hastanın 51’inde (%71,83) genotip 1b gözlendi. HCV-RNA pozitif olan örneklerin geri kalanında; 6 hastada genotip 1a (%8,45) ve 8’inde (%11,27) genotip 2b enfeksiyonu gösterildi. Geri kalan 6 örneğin 3’ünde (%4,22) genotip 1 ve 3’ünde (%4,22) genotip 3a olarak tespit edildi. Genotip ve ya alt tip dağılımı bakımından kadın ve erkek cinsiyet yönünden istatistiksel olarak anlamlı bir fark gözlenmedi.
Sonuç: Sonuçlarımıza göre genotip 1b Adıyaman ilinde baskın olarak saptandı. Ancak zamanla diğer genotiplerinde ortaya çıktığı gösterilmiştir.
